**PLEASE READ CAREFULLY**
All students participating in Athletics, Band, Cheerleading, Drill Team, ROTC, and Club Sports for the 2022-2023 school year at The Woodlands High School will be required to complete a new athletic physical for the 2022-23 school year.
In a continued effort of CISD to keep paper use to a minimum, all students that will participate in activities that require a UIL physical - Band, Cheerleading, Drill Team, ROTC, Athletics, and Club Sports for the 2022-2023 school year at The Woodlands High School must complete their UIL paperwork online. The online forms are required. Parents should complete the online forms and turn in a hard copy of the 2022-23 Medical History and Physical Exam Form to the Coach or Athletic Training Staff before the student participates in his/her activity.
The on-line program that has been approved by Conroe ISD is Rank One. The registration will only take a few moments. Below is the link to the instructions for you to follow to complete your student’s information. This information can be completed on a computer, tablet, or smart phone. If your student will participate in one of the above listed activities you can register as soon as you would like and begin completing the forms.
