TWHS Band Booster Handbook

TWHS Band Agreements
The following forms must all be completed by the Band Member, the Band Member's Parent, or both where indicated, before the Band Member may participate in any TWHS Band activities.

Physicals
For the 2019-2020 school year Conroe ISD requires ALL students to complete this 2019-2020 Conroe Independent School District Pre-Participation Form. Please click on the link below to download the form from the CISD web site. Please note that CISD will not accept physicals or completed paperwork dated prior to April 15, 2019.
If your son/daughter will be participating in Athletics, Band Cheerleading, Drill Team, ROTC or TWHS sponsored club sports they are required to have a completed physical on file in the Athletic Training Room at The Woodlands High School.
All students participating in the above listed UIL activities will need to complete the UIL required forms ONLINE. The student will also need to get a physical and turn the form in to the Athletic Training Room.
To expedite this process, Highlander Sports Medicine will use an online data entry program called SportsWare. All data must be entered by a parent or legal guardian of the student.
